Horizon is huge in New Jersey — but "I have Horizon" isn't an answer
Horizon Blue Cross Blue Shield of New Jersey covers more state residents than any other insurer, so it's the card our callers mention most often. But Horizon sells many different plans — employer plans, marketplace plans, Medicare Advantage, and managed Medicaid through NJ FamilyCare — and each one handles substance use benefits a little differently. Two people holding a Horizon card can face completely different deductibles, networks, and authorization rules for the exact same detox stay.
That's why the useful question isn't "does Horizon cover rehab?" It's "what does my specific Horizon plan cover, at which facilities, with what out-of-pocket cost?" Everything below is about getting that answer quickly.
What substance use benefits usually include
Under federal mental health parity rules, plans that cover substance use treatment generally can't impose stricter limits on it than on comparable medical care. In practice, Horizon plans commonly include benefits across the main levels of care:
- Medical detox — short-term, medically managed withdrawal, usually in a licensed facility.
- Residential or inpatient rehab — living at the facility for a structured program.
- Partial hospitalization (PHP) — full treatment days while sleeping at home.
- Intensive outpatient (IOP) and standard outpatient — scheduled sessions around work or family life.
- Medication treatment for opioid or alcohol use disorder, subject to the plan's pharmacy and medical rules.
"Included as a benefit" doesn't mean "free" or "automatic." Deductibles, coinsurance, and out-of-pocket maximums still apply, and higher levels of care almost always require the plan to agree the care is medically necessary.
Prior authorization: the step that decides most admissions
For detox and residential care especially, Horizon plans typically require prior authorization — the facility submits clinical information and the plan approves a specific number of days. A few things worth knowing:
- The facility usually handles the authorization paperwork, not you — but you can and should ask whether it's been approved before admission day.
- Approval is often for an initial block of days, with continued-stay reviews after that. Ask how the program handles it if the plan authorizes fewer days than the clinician recommends.
- A denial is not the end. Ask for the reason in writing and whether the program will appeal or request a peer-to-peer review between their clinician and the plan's reviewer.
How to verify your coverage in one phone call
The member services number is on the back of your Horizon card. Have your member ID ready and ask these questions in order:
- What are my substance use disorder benefits for detox, residential, PHP, IOP, and outpatient care?
- What is my deductible, and how much of it have I met this year? What is my out-of-pocket maximum?
- Is the facility I'm considering in-network for my plan? (Give the exact facility name — networks differ between Horizon products.)
- Is prior authorization required, and who submits it?
- Can you send me the benefit details in writing or through my member portal?
If you'd rather not make that call cold, call our line first at (862) 369-7533. We can't see your plan and we don't bill insurance, but we can help you get the questions in order, explain what the answers mean, and talk through rehab placement options that fit what you learn. That call is free and confidential.
A clear word about who we are
Parsippany Detox Assistance is an independent information and referral resource. We are not affiliated with, endorsed by, or paid by Horizon Blue Cross Blue Shield of New Jersey, and we don't represent any insurer's interests. Benefit decisions belong to the plan; treatment decisions belong to licensed clinicians. Our role is narrower: helping Morris County callers understand the landscape, ask sharper questions, and find licensed programs to compare. Always confirm network status and costs directly with Horizon and the facility before you commit.
Frequently Asked Questions
Does Horizon BCBS cover drug and alcohol detox in New Jersey?
Most Horizon plans include some behavioral health and substance use benefits, but the details — deductible, copays, which facilities are in-network, and whether prior authorization is required — depend on your specific plan. The only reliable answer comes from verifying your own plan, either through the member number on your card or with the facility's admissions team.
How do I check what my Horizon plan will pay for?
Call the member services number on the back of your card and ask specifically about substance use disorder benefits: detox, residential, partial hospitalization (PHP), intensive outpatient (IOP), and outpatient. Ask what your deductible and out-of-pocket maximum are, whether the facility you're considering is in-network, and whether prior authorization is needed. If you'd like help preparing for that call, our line at (862) 369-7533 can walk you through the questions.
What if Horizon denies coverage for a stay?
Denials can be appealed. Ask for the denial reason in writing, and ask the treatment program whether they handle appeals or peer-to-peer reviews with the insurer. Many denials are overturned or resolved with additional clinical documentation. You can also file a complaint with the New Jersey Department of Banking and Insurance if you believe a denial was improper.
Is Parsippany Detox Assistance part of Horizon's network?
No. We are an independent information and referral line, not a treatment provider, and we have no affiliation with Horizon BCBS or any insurer. We don't bill insurance, and we can't tell you what your plan will pay — only the plan and the facility can confirm that.
Can you help me find a rehab that takes Horizon?
We can help you understand what to ask and how to compare licensed programs, and many callers use our line to talk through placement options before they commit. Network status changes, though, so always confirm directly with both the facility and Horizon that a program is in-network for your plan before admission.
